Abstract

Problem/ Purpose: Postoperative blood glucose (BG) monitoring in the post-anesthesia care unit (PACU) is critical for preventing complications such as hyperglycemia, hypoglycemia, surgical site infections, and prolonged hospital stays. Internal data from 2023 revealed that only 60% of diabetic surgical patients received BG monitoring within 30 minutes of PACU admission, leaving 40% at risk for undetected glycemic events. This quality improvement (QI) initiative aims to enhance PACU nurses' adherence to postoperative glucose monitoring guidelines by implementing a structured checklist during patient handoff, ensuring timely BG checks. Method: This 15-week project engaged 23 perioperative nurses and 10 anesthesia providers. Education was provided through PowerPoint presentations and email follow-ups. Preoperative nurses attached a checklist to diabetic patients' face sheets, serving as a visual reminder for PACU nurses to complete BG checks within 30 minutes of admission. The project lead (PL) conducted 10 random weekly chart reviews, and PACU nurses received weekly surveys to assess checklist utilization. Results were shared with stakeholders, including the PACU team, Clinical Site Representative (CSR), and perioperative manager. Results/ Conclusions: Checklist utilization increased steadily over 15 weeks, starting at 2 out of 5 nurses in Weeks 1-2 and reaching full adherence (5 out of 5) in most weeks from Weeks 10-15. Peaks in Weeks 4 and 9 were linked to increased staff rotations. Chart audits of 137 patient records showed 99.2% compliance. The intervention improves PACU nurses’ adherence to BG monitoring, achieving 99.2% compliance. It is cost-effective, sustainable, and scalable, with the potential for EHR integration and expansion to other perioperative settings.
